FILE A CLAIM

Pursuant to the Court's Order filed on August 31, 2022, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim form ("Claim Form" or "Proof of Claim") in accordance with the instructions contained therein. To obtain a copy of the Claim Form, click here. The deadline to submit a timely Claim Form is December 19, 2022.

If you fail to execute and submit a timely, properly completed and addressed Claim Form, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
